  "short_answer": "If someone develops **new sudden confusion**, contact the local emergency medical service immediately. Sudden confusion alone is enough; do not wait for another symptom or a judgment that it is “severe.” If **poisoning is suspected**, make that contact immediately, even before symptoms appear. Dangerous breathing problems also need emergency medical help. Do not spend this time proving a cause, searching for testimonials, or finishing a home experiment. The [NHS page on sudden confusion](https://www.nhs.uk/symptoms/confusion/) and [NHS page on poisoning](https://www.nhs.uk/conditions/poisoning/) describe UK routes; check the appropriate service and number where you are.",

Yet three further claims can slip in beside it: “this particular remedy caused the improvement,” “it treats addiction,” and “it will help other people.” The initial account reports experience; the next proposes a cause; the others require a specified outcome and evidence that it applies elsewhere. A truthful narrator cannot make these different claims equivalent.\n\nConsider a **hypothetical example**, not anyone's biography: after a difficult week, a person sleeps better while also changing their schedule, receiving a friend's support, and starting a new approach. There is no need to deny their relief. This one case cannot separate the new approach from sleep, time, support, and changed circumstances. Nor can we confidently dismiss the improvement as *only* “placebo.” A useful study asks a more precise question.",

      "q": "What, exactly, was measured?",
      "a": "“I feel better” and “a test score changed” are different outcomes. Either may matter if we know what was measured, when, by whom, and for what purpose. Less anxiety today does not automatically establish fewer relapses later; the number of tasks completed does not measure errors or road safety. The [NCCIH page on study results](https://www.nccih.nih.gov/health/know-science/how-to-make-sense-of-a-scientific-journal-article/results) points readers to the importance of an outcome to people, replication, and comparison with earlier work. One experience or one study seldom settles every question.\n\nNumbers also differ from the explanation given for them. [NCCIH describes a study's discussion](https://www.nccih.nih.gov/health/know-science/how-to-make-sense-of-a-scientific-journal-article/discussion-conclusion) as the authors' interpretation of results, which other researchers can challenge. Read a conclusion beside its methods and data. There is no reason to assume, by default, either that researchers are lying or that every maker of medicines is part of a conspiracy.",

      "q": "Why comparison matters without settling everything",
      "a": "To ask whether an intervention caused a particular effect, we need a way to examine alternative causes. [NCCIH explains comparative study design](https://www.nccih.nih.gov/health/know-science/how-to-make-sense-of-a-scientific-journal-article/methods/design-of-the-study): groups and, in some studies, blinding can help. But the label “randomized trial” does not guarantee that it measured the outcome you need, included people like you, or followed them long enough. A missing randomized trial also **does not prove the absence of an effect**. The question is what is known, for whom, and with how much uncertainty.\n\nThe [NCCIH overview of research types](https://www.nccih.nih.gov/health/know-science/how-to-make-sense-of-a-scientific-journal-article/methods/types-of-research) distinguishes methods, participants, measures, and ways to reduce bias. Carefully conducted qualitative interviews can be especially useful for learning what people value, which barriers they face, and what standard measures miss. The [FDA's patient-focused guidance-series page](https://www.fda.gov/drugs/development-approval-process-drugs/fda-patient-focused-drug-development-guidance-series-enhancing-incorporation-patients-voice-medical) recognizes patient input in medical product development and discusses qualitative methods. This is a US methods landing page, not a claim that every full FDA guidance was read or that one testimonial proves effectiveness. Describing experience and making a causal comparison answer different questions; neither makes the other unnecessary.",

Accuracy of work, memory, response to an unexpected event, driving, and control over use are different outcomes. A pleasant feeling or a self-description as “strong-minded” cannot establish them.\n\nThe US [CDC page on cannabis and driving](https://www.cdc.gov/cannabis/health-effects/driving.html) describes possible impairment of reaction, coordination, and decision-making. Crash research shows associations, while a THC reading does not precisely measure an individual's impairment. None of this supplies a personal “safe hour” based on how one feels. If there is doubt about driving or doing a hazardous task safely, postpone it and arrange an actually capable adult driver or replacement. The [CDC page on cannabis use disorder](https://www.cdc.gov/cannabis/health-effects/cannabis-use-disorder.html) explains that it can occur among people who use cannabis. Satisfaction with one's work provides no immunity, but it does not establish a diagnosis either.",

How long was the follow-up, and who dropped out? What else changed at the same time? Who is reporting an experience, who is reporting a measured result, and who is proposing a cause? These are not a scored questionnaire and do not require you to become a researcher. They can keep a single case from becoming a universal promise.\n\nYou can take your own story to a clinician or local support service: “I noticed this change, and I use these medicines and substances. Can you help me distinguish a possible benefit, risks, and available options?” Ask about language, cost, waiting time, consent, and use of your data. A referral or appointment request does not yet mean care actually happened. Do not dismiss prescribed treatment because of someone else's testimonial, or stop or change it on your own. Physical adaptation to a prescribed medicine is not, by itself, a substance use disorder. Do not conduct withdrawal at home to test a theory.\n\nIf there are threats or violence, seek a separate safe route for support, without requiring a meeting with the person posing the threat. A child must not monitor an adult, translate for them, or manage medicines. If an adult cannot care for the child, an **actually present and capable adult** must take over, not merely a phone number. Check local rules and actual availability for consent, confidentiality, and child care. Emergency medical contact does not wait for answers to research questions.",
