# Leaving an institution: care continuity and a life of one’s own

Release or discharge does not mean accommodation, documents and a receiving appointment are already available. This article distinguishes actual help, accepted work conditions and remaining gaps. Continuity matters beyond addiction, while adult voice remains alongside helper limits and children’s safety.

For adults leaving custody, a closed treatment facility or hospital, relatives and organisers; settings have different rules and departure assigns no diagnosis.

- Where can the person actually stay today?
- Which documents are available and who answers for missing ones?
- Who will actually continue prescribed care?
- What work terms have been accepted and what remains open?
- Which help was received and which merely promised?

No local legal instruction, medicine choice, home detox, compulsory whole-history disclosure or dangerous test. Suspected poisoning should not await symptoms; new sudden confusion itself calls for immediate local emergency medical contact. Threats need a suitable safe protective route. Access, method and safety are unestablished; inability is no fault. A child is no interpreter or rescuer and actual care duties remain.
