## Content

For an adult after an established stroke or acquired brain injury and a supporter they choose. The article distinguishes movement, speech, understanding, memory, attention, and conditions of participation. A similar difficulty does not establish the same cause or justify transferring another person's diagnosis, prognosis, or exercises. This is about questions for support, not a rehabilitation programme.

## Main points

- Slow speech does not establish absent understanding. Confident speech does not establish access to every complex task. Individual assessment matters more than either assumption.
- An observation, an inferred motive, and a medical conclusion are different information. Silence cannot be filled with consent.
- A response method and communication aid need to suit the person. The article does not prescribe a universal nod or app.
- Support goals should include activities meaningful to the person without guaranteeing achievement or morally scoring an outcome.
- Home and family are not assumed to be a ready-made service. Missing money, equipment, or adult support remains a real barrier.
- A philosophical question about identity is not a diagnosis or grounds for withholding respect.

## Questions and answers

**How can a difficulty be described?** Name the observed situation without judging character: for example, a response does not follow several tasks being explained together. Distinguish professional findings from what remains unknown.

**How can communication help be requested?** Ask who can assess a suitable way of explaining and responding. Conversation partners may need training too. The article neither selects a device nor prescribes exercises.

**What is possible without an app or relative to help?** One accessible spoken question to the relevant participant can begin. A private centre is not assumed. An absent resource is not a personal failure.

**Can someone close decide what the person wants?** Their observation differs from the person's confirmed position. An unclear response method remains unknown. Local authority requires separate checking.

**When must reading give way to urgent care?** New sudden speech difficulties, weakness, confusion, or other concerning acute symptoms need urgent attention. Symptoms passing does not establish a safe wait. Emergency help comes before reading.
