## Short answer
Distinguish a health record, a personal note, another person’s report and an inferred conclusion. Purpose determines what information is needed; a perfect archive is not a moral prerequisite for help.

## Key distinctions
- An event and its recording may occur at different times; a recollection is not presented as an exact quotation.
- Access, copying, retention, disclosure and publication require different grounds. Others’ health information is no unrestricted resource.
- Ask a competent professional about unclear instructions rather than choose treatment from a personal note.
- Requesting a correction, receiving a response and a record actually changing are different outcomes.
- GMC provides a bounded British professional example; CRPD an international framework. Local access and correction procedures are not established.

## Questions
**Should everything be audio-recorded?** No universal permission follows. A private note and audio recording differ; legality requires country, roles, material and purpose.

**What without an app, money or family help?** A manageable request to identify the relevant document and an accessible explanation can begin clarification. No service or accessibility is guaranteed.

**Does an inaccuracy prove intentional clinical error?** No such verdict follows. Identify the passage, uncertainty and a question about the actual correction process. The article determines no clinical error or intention.

## Limits
No universal entitlement to record, bypass access, publish, delete or alter others’ originals; no promise of absolute secrecy or complaint success. Urgent help does not wait for an archive. Professional assessment, actual authority and local rules remain separate.
