## Purpose

For an adult offered DBT or other structured care: understand actual components, agree a goal, contact arrangements and benefit review. Conversation preparation rather than a home DBT course or method choice through self-diagnosis. GEO is subordinate to this master.

## Claims

NIMH describes DBT among BPD approaches; strong feelings alone do not diagnose BPD. Comprehensive DBT in Linehan 2015 included individual therapy, skills group, between-session telephone coaching and a therapist consultation team. A group or DBT-informed format is not the entire programme; limited scope does not automatically establish uselessness. A service needs honest explanation of aims, limits and risk arrangements.

APA's official 2024 summary supports research-supported structured psychotherapy and a documented collaborative person-centred plan without one universally best method. The eight-recommendation summary was read, not the full guideline. The 2015 trial studied 99 women with BPD/high suicide risk, a treatment year and follow-up, differing components and risk-trained clinicians in all groups. Improvement and some component advantages cannot establish guarantees, home-exercise equivalence or safe removal of any component.

Goals concern meaningful daily life and safety rather than constant calm. Understanding, application, life changes and safety differ. Attendance, referral, waiting list and benefit are different events. Physical health, co-occurring conditions and medication need appropriate assessment; prescriptions are not independently changed. Deterioration can be raised before course completion; no universal success deadline or instruction to abruptly leave care.

## Conditions and practice

An optional note: “I want to change…; the obstacle is…; I want to ask….” One spoken question about components, a goal, contact or review can start. Limited-resource circumstances include cost, language, transport, waiting, lack of phone or privacy. Access does not measure motivation. Alternative structured care/support while waiting can be discussed without promising places.

Coaching is not continuous personal access. Hours, channels, purposes, response time and danger routes need clarification. Sending a message proves neither reading nor immediate help. Immediate danger/inability to maintain safety requires available immediate assistance rather than waiting. Language does not determine country; foreign numbers are not exported. Relatives have agreed roles, children are not dispatchers. Confidentiality requires local checking. Acceptance of dignity excuses no violence; change requires no humiliation.

## Traditions and sources

Aristotle II.1–2: learning through action and circumstances, not DBT efficacy/dose evidence. Seneca II.3–4: involuntary onset and subsequent action, not universal neural mechanism or total control. Reciprocity in Legge XV.XXIII questions arrangements on both sides without replacing contracts or law.
