# Moving to adult mental health care

## Main answer

Continuing need, a suitable receiving service and an actually accepted task are different facts. Referral sent, document received, appointment arranged and care started each need their own confirmation. One system's age threshold is no universal day of independence. Diagnosis alone does not remove a person's voice; family assistance does not require handing over every decision.

## Example

In a hypothetical story, seventeen-year-old Kirill prepares to move services. He wants to speak for himself but does not know who answers between teams. His parent fears losing any involvement. Two small questions offer a beginning: who currently considers continuing needs, and which particular assistance does Kirill want from a chosen adult? Speaking during an appointment and requesting travel assistance can coexist.

## What to clarify

Ask an existing responsible professional which next contact is actually confirmed, what has reached the new team and who handles current questions until care continues. If an individual plan exists, ask the team how it applies after transfer; this article does not rewrite it. Unclear prescriptions need a professional response. This article cannot authorize changing doses, extending or stopping medicines, making up a missed dose or choosing replacements.

Distinguish parental attendance, organizational support, reporting observations, access to information and decision-making authority. Consent, confidentiality and exceptions depend on age, territory and circumstances. Before sensitive discussion, ask who may receive information and under which conditions. A general article establishes neither absolute secrecy nor automatic family disclosure.

## With limited resources

Naming one unfinished step can suffice: the letter was sent but the next contact remains unknown. Personal notes are optional and cannot replace clinical documentation. Lacking money, a phone or reliable memory does not make a need less substantial. Ask a service about accessible communication and actual support; neither free resources nor a particular deadline are guaranteed here. The young person is not assigned coordination of the whole system. A teenager is not responsible for reconciling adults or managing medical care.

## Limits

Abrupt severe deterioration or immediate danger requires urgent local medical involvement without waiting for completed transfer, a first appointment or agreement about whose responsibility this is. Family support does not replace an absent service.

Selected main text from [NHS](https://www.nhs.uk/mental-health/children-and-young-adults/mental-health-support/how-to-get-support/moving-on-to-adult-services/) was read in an expanded official search result; direct opening failed. Main sections from HSE on [the first appointment](https://www2.hse.ie/services/moving-from-child-to-adult-health-services/your-first-time/), [preparation](https://www2.hse.ie/services/moving-from-child-to-adult-health-services/planning-your-move/) and [moving services](https://www2.hse.ie/services/moving-from-child-to-adult-health-services/why-we-move-you/) were checked. British/Irish ages and organizational rules are not transferred worldwide. NICE is currently unavailable; exact previous reading remains historical evidence. Dewey IV.1, Epictetus1 and Seneca9 help distinguish participation, external outcomes and relationships without determining treatment or local law.
