What matters to me at the end of life: a conversation in advance

Guide contents

Discuss one personal priority without settling the whole future. Distinguish the person’s words, a helper’s inference, clinical questions and a note’s status. Fear of being a burden is no consent to less treatment; age, diagnosis, disagreement and silence confer no universal authority. A pause does not establish inability to participate. An informal note does not itself become treatment refusal, a resuscitation decision, legal appointment or will. Clarify the applicable process professionally. Do not remove food, care or relief for an answer, independently alter treatment or delay current urgent help.

Questions explored

Is an advance conversation already a treatment refusal?

No. Priorities do not settle a particular intervention; a personal note is not declared a legally effective instruction. Discuss clinical questions and applicable processes separately.

May someone not know the answer yet?

Yes. Clarify whether a pause, other format or explanation is needed. Promise no unlimited time, but do not use uncertainty to extract an answer or take universal authority.

What does not wanting to be a burden mean?

One phrase cannot establish that. It may concern costs, loved ones or treatment by others. Ask its personal meaning rather than assuming consent to less treatment and help.

Key distinctions

Advance conversation can concern wishes without establishing a death date or specific medical decision.

  • Fictional Nina/Vera, no clinicalcase/prognosis/guaranteed benefit or transferred British process.

Values, specific wishes and necessary conditions differ; fear of being a burden needs clarification.

  • No consent to less treatment from a broad phrase, household capacity test, removal of support for a clean answer or universal authority transfer.

Conversation and postponement remove no necessary help and establish no inability to participate in decisions.

  • No independent treatment changes/foodcare relief denial, pressure/provocation/faith prerequisite or absolute secrecy; urgent help does not await discussion.

NHS distinguishes an advance statement and advance decision; a family note is not declared to have every medical or legal effect.

  • Version2023,nextreview2026passed; current validity/signatures/witnesses/resuscitation/appointment/will not determined, no legaltemplate.

Clarify the meaning of change and current understanding; discussion need not complete relationships, memory projects or every question.

  • No endless consistency questioning, forgiveness/unsafe contact duty; amending a note confirms no formal document amendment.

Ecclesiastes and Epicurus are compared through limited command of time while retaining different grounds.

  • No dyingcalendar/prognosis, violence/caredenial/suicide advice, patientblame or faith/fearlessness prerequisite.

Limits of interpretation

  • NHS C1/C2 reviewed2023,nextreview2026passed; no newer version established or current legal validity confirmed.
  • No clinicalcase/prognosis/interventionchoice/urgency scale/consentform/legaltemplate or ADRT/DNACPR/LPA/will procedure.
  • Words/inference/medicalpossibilities/serviceconditions/legalstatus are distinct questions.
  • Age/diagnosis/disagreement/silence confer no universal authority; no household capacity test/provocation/support removal.
  • No independent treatment change or food/care/relief denial for answers; current urgent help does not wait for discussion.
  • A pause establishes no incapacity, later words not automatically free/understood; documentary amendments need separate clarification.
  • No absolute secrecy/universal service/implementation guarantee, endless helper duty or compulsory memory/forgiveness/reconciliation/unsafe contact.
  • Ecclesiastes/Epicurus are no clinical/legal grounds, dying schedule, violence/suicide/treatment-refusal instruction or dignity verdict.
