Questions & Answers

A diagnosis has been revised: what changes, and what does not disappear?

Questions & Answers

Based on LHD-016,r01,ARTICLE_EN.md.

What can be clarified?

Which statement changed, its grounds and the new explanation's status. Revising a cause does not erase observed experience or automatically prove a new cause (I–II).

What about care and documents?

Ask separately what changes, who considers conflicting instructions and where current explanation is recorded. No independent record amendment or treatment change is proposed. A file sent does not establish consideration (III–IV).

Does change prove error?

The article draws no such conclusion. New evidence, clarification and possible error require different consideration;legal assessment depends on location (V).

Limited resources and boundaries

A spoken account of a continuing need and one next-step request can begin. Accomplished acceptance and a public self-revision are unnecessary. No safe waiting period is given;serious current danger calls for urgent help (VIII,practice).
