A course of assistance has ended: arranging the next transition

Answer

A service ending, continuing need and assistance actually continuing are different states. Clarify what ended, applicable individual instructions, a receiving participant, a confirmed next contact and unresolved conditions. A referral letter is not an accepted request; a contact is not assistance received.

Argument

I identifies what ended without turning an administrative boundary into a medical conclusion. II keeps need visible without endless pursuit of improvement. III distinguishes sending, receipt, response and help. IV connects documents to an understandable purpose and an accessible personal response. V clarifies helper limits and costs without promising extension rights. VI allows a farewell's significance without demanding personal friendship from staff. VII compares Stoic self-sufficiency with Gospel continuity of care while retaining different grounds and the injured person's underdeveloped voice. VIII allows an honestly unfinished transition and life outside a recovery project.

Application

Choose one question: what ended, what applies now, and where should continuation be addressed? Separate confirmed from proposed steps. Agree one manageable action with a chosen helper. Without money, family or convenient writing, one spoken request for assistance finding a recipient is possible. No service is guaranteed; without a recipient, a gap remains. A child does not replace an absent adult, and agreeing to a call does not appoint a friend coordinator of an entire life.

Sources and limits

WHO Rehabilitation 2024 concerns functioning, environment and goals. Current NHS Planning to leave hospital, reviewed 8 April 2026, is an English transition-discussion example rather than worldwide procedure or a guide to ending every course. Seneca 9 distinguishes moral good from means of existence; Luke 10:30–37 shows continuation with another participant without a modern consent protocol.
