Questions & Answers

Insurance coverage: a promise, a contract and actual payment

Questions & Answers

Direct answer

Advertising, a current contract, prior authorisation, a claim decision and actual payment are different results. Clarify the service, plan, conditions and authorised recipient. Holding a policy does not confirm payment for every expense.

Example

A clinic promises fictional Nina that insurance will pay everything. This does not yet explain insurer confirmation, what the service includes and any personal share. The promise need not immediately be declared false; identify its exact subject and basis.

Distinctions

Who pays whom: the provider, or reimbursement to the person? What is authorised, pending, and confirmed as paid? Bills, benefit explanations and an already paid personal share differ. An unknown balance is not declared final debt by this article.

If unpaid

Refusal, delay, an unreceived claim and an information request need different questions. Clarify the basis, actual local review route and deadlines. Discussion does not automatically suspend time. Do not change facts, dates and documents to obtain cover.

Resources and safety

Expected reimbursement is not present money; credit, family help and paid representation are not assumed. A helper retains the adult's voice; a child should not coordinate medical and insurance affairs. Immediate danger or serious deterioration requires appropriate available local urgent help without waiting for an insurance file. Free access and coverage are not promised.
