Short answer

Choose one meaningful available conversation, a specific task or declining contact. Apology, forgiveness, reconciliation, trust and resolution of a practical matter are different events.

Questions

Does serious diagnosis require immediate reconciliation? It alone establishes no prognosis and makes no contact compulsory. Can a practical matter be passed on without restoring a relationship? They differ; necessary procedures/powers need separate checking. What if the other person does not answer? An attempt guarantees no response; incompleteness does not determine life's value. Without money, family or an app? One oral request at an available encounter is possible; an intermediary and safety are not promised.

Limits

No therapy, forgiveness guarantee, legal form or compulsory meeting with someone harmful. A perfect evidential history is not required to decline contact. Necessary care and urgent help do not wait for reconciliation. Disability does not mean dying.
