Question and answer

How can changes in sleep, mood, and perception be discussed alongside substance use? Do not choose one everyday explanation beforehand. Bipolar disorder, psychotic experience, use, substance effects, and substance use disorder differ; questions can coexist. History and professional assessment matter more than one night's impression. Afterwards does not prove because of.

Application

Communicate available history: change from usual, timing, known substance/medicine/physical information, and present need. Observation, another's account, documents, and inference remain separate. Unknowns need no invented explanation. There is no home diagnosis, covert testing, password gathering, or independent treatment discontinuation. Prescribed medication alone does not establish substance use disorder.

Clarify who assesses possible causes, connects care, and accepts the next task. ASAM/AAAP's concurrent-treatment recommendation concerns stimulant use disorder and co-occurring conditions; it is strong with very low certainty. It is not extended to every substance class or converted into a reader's treatment schedule.

Distress and meaning can be heard without confirming unusual content or debating reality. Culture and faith deserve respect; spiritual explanation does not replace medical help. A relative's support has actual agreed limits. Abuse requires a separate safe route without compulsory reconciliation; diagnosis does not prove violence.

Urgency and access

New sudden confusion itself requires immediate local emergency medical help. Suspected poisoning requires immediate contact even without symptoms. Severe withdrawal in physical alcohol dependence requires urgent help; the risk is not assigned to everyone who uses alcohol. Immediate danger to self/others or inability to meet basic needs safely must not wait for reading or family agreement. There are no home detox instructions, dosages, covert treatment, or force instructions.

A child or dependent person missing necessary care needs an actual capable adult or professional. Messaging a service does not replace present help or delay medical contact. A child is not the dispatcher or symptom observer.

Without money, a telephone, or safe family, one question at an available in-person contact is possible. Finding a number, sending a message, referral, and received care differ. Language, cost, consent, disclosure, and local exceptions require actual verification; no universal system or absolute secrecy is promised.

Evidence and provenance

NIMH Bipolar Disorder, NIMH Psychosis, NHS confusion, poisoning, alcohol, and selected ASAM/AAAP recommendation21 support bounded master claims. Other exact inherited research scopes and links are retained in SOURCES.json; complete guidelines and underlying studies are not claimed read.

Epictetus XLV, Plato's Phaedrus244–245, Kant II, and Job6 are compared with explicit limits: moral judgment is not diagnosis, divine madness is not contemporary psychosis, and compassion does not require enduring abuse. This adapts DEP-18-N05 revision1 under new MHR-055, preserving the original without transferring its acceptance. Clinical, native-language, human-outcome, coordinator, and production acceptance remain NOT_RUN.
