After discharge: keeping care connected between two services

Questions & Answers

Purpose and urgency

MHR-083/r01 concerns adults after care for a mental health crisis. The end of a stay, disappearance of every need and actual continuing care differ. This article neither determines readiness for discharge nor changes treatment. Danger to life, serious injury or inability to maintain safety needs appropriate local emergency help. Sudden confusion requires immediate medical assistance, without waiting for records, a coordinator or a complaint.

Establishing handover

Sending records, receiving information, accepting the next task and accomplishing contact are distinct. Establish the receiving team, confirmed next step, communication and who can help if arrangements fail. If no receiver is found, the accurate status is that care is not connected yet. This is not necessarily refusal by the person or a legal finding of fault.

A daughter, relative or phone number does not prove coordination. A patient need not master inter-agency procedures or navigate every inaccessible entry point before asking for organizational assistance. Convey the actual barrier when strength, language, money, housing or communication is absent. Alternatives must be established rather than guaranteed.

Access and roles

A prescription, understandable instructions and actual supply differ. Contradictory old and new records, including physical and psychiatric care, need professional clarification. There is no independent choice of compatibility, dose, substitution or discontinuation. An old diagnosis does not automatically explain a new condition.

Travel, meeting, entry and everyday support are separate transition tasks. Do not test them through dangerous action. A promised call is not daily care; family membership creates neither continuous availability nor representation. Clarify an agreed role, resources and authority separately. Needing assistance does not erase privacy; absolute secrecy obstructing safety is not promised.

Name a known gap before failure. A complaint may investigate the past without meeting today's need. Do not provoke danger or withdraw necessary care to establish evidence. Temporary arrangements retain stated limits. Early days and changed needs permit review without demanding grateful cheerfulness or complete independence.

Comparisons and sources

Laozi63–64, Legge: caution about easy promises and attention at the end. Luke14:28–33, KJV: conditions for continuing within teaching about discipleship, not a price of life. Galatians6:1–5: mutual participation and individual responsibility, without treating illness as moral fault. These distinct traditions are no protocol. Daoist non-action does not justify services disappearing; burden-sharing does not make a daughter the entire care system.
