Planning for worsening mental health: my preferences, people and available help

Purpose and urgency

MHR-084/r01 helps adults discuss preferences, chosen people and actual assistance in advance. A plan neither predicts crisis nor guarantees prevention or replaces individual assessment. Danger to life, serious injury or inability to maintain safety requires appropriate local emergency help; sudden confusion needs immediate medical assistance. Do not wait for a perfect plan or every relative's agreement.

Four different results

Personal preference, an agreed supporter role, a clinical recommendation and a possible legal document differ. A notebook gains neither clinical nor legal authority from its title. Previous helpful experience can be described as experience without proving universal adequacy. Discuss individual medical actions and their limits with an appropriate professional. The article supplies no doses, safe waiting periods or home scale.

Maryland Department of Health describes an advance directive within one jurisdiction. This example determines neither a reader's document validity nor priority during conflict or guaranteed outcome. Forms, app and linked laws were not examined. Local requirements need separate clarification. CRPD12/21 supplies supported-participation and communication norms, not individual powers.

People, records and access

A chosen supporter does not acquire every decision. Establish the function, agreement, ability and limit. Slow or absent speech does not automatically transfer the person's voice to family. Participation and information sharing need specific subjects, privacy and actual rules; absolute confidentiality is not promised.

A number, delivered message and clinical answer are distinct. A routine channel is no emergency route. Examine record access safely: an app-held file without communication is not accessible. GMC65 is a bounded British standard on information and responsibility, not automatic worldwide exchange. No public distribution or hazardous rehearsal is proposed.

Without family, money, an app or backup, begin with one question to an available participant. An unconfirmed name is not an accepted role. An absent resource is not personal disorganization. Changed preferences, availability and clinical questions differ; editing a number does not cancel prescriptions. Conflict between an earlier record and a present position requires suitable local clarification rather than silent priority for paper.

A plan is no obedience contract or promise that crisis will never return. Do not punish by withholding essentials or create danger to test arrangements. Ordinary disagreement becomes no emergency through a supporter's inconvenience; actual danger need not wait for a routine contact chain.

Comparisons and limits

Proverbs18:13–17, KJV: listening before answering, without automatic family-account priority. Epictetus I, Higginson: participation and uncontrollable outcome, without willpower control of symptoms. Their grounds differ and do not collapse into advice to calm down. Philosophy does not remove actual organizational responsibilities.
