Reader and question

An adult already receiving psychotherapy who cannot tell whether it helps. The task is to discuss the goal, changes and burden, then consider continuation or review. Immediate improvement is not demanded; harm is not excused through a universal account of resistance.

Distinctions

Unclear goals, hard-to-notice change, discomfort, deterioration and boundary violations call for different questions. Difficult feelings prove neither success nor harm. A change following an appointment does not establish its cause. Stability may be an agreed support goal without itself proving an effect. Mood, symptoms, everyday participation and self-understanding are distinct.

Accessible application

Choose one example: what happened, why it matters and what needs clarification. Ask to use part of an appointment to discuss the link between current work and the goal. With enough energy, choose one sign of change and one sign of burden; with little energy, a sentence about unclear benefit is enough. No diary, safe device, relative or money for a second opinion is assumed. Inadequate access remains an unmet need.

Limits

Substantial deterioration calls for suitable assessment; immediate danger calls for available local urgent help without waiting for a conversation or report. The article determines no symptom cause, gives no medication changes and sets no effectiveness deadline. Unsafe contact does not require reconciliation or a prior complaint. Specific misconduct and local reporting arrangements need separate examination. MHR-M03 and MHR-L01–L07 are internal navigation rather than published services.

Grounds and attribution

APA psychotherapy information supports discussion of goals, limited progress and duration; no general promise that deterioration means growth is adopted. VA/DoD 2022, recommendation 2 and section C, concern measurement in depression treatment, not a universal scale. APA §10.10 concerns professional responsibility for continuation and ending rather than worldwide law. Dewey and 1 Thessalonians 5:19–22 offer distinct accounts of examination and retaining what is good; philosophy and religion are not clinical evidence.

Understanding supported

Someone can retain what helped while naming an unresolved need. Review may include real changes to the goal, pace or format, additional support, transfer or ending. Availability of alternatives and safety of waiting are not guaranteed.
