Reader and benefit

For an adult offered ECT or rTMS. Helps clarify indications, alternatives, consent, risks and actual care arrangements. These are different interventions. Another next method needs its exact name and a specific source. No procedure choice, settings, home device or outcome guarantee is supplied.

Distinctions

ECT induces controlled seizure activity under general anaesthesia with muscle relaxation and trained monitoring. rTMS uses repeated magnetic pulses, generally without general anaesthesia; assessment and risk remain. Reasons for a proposal differ: severity, previous response, tolerability or urgency. VA/DoD2022 MDD recommendations17/20 differ in scope and strength; they are neither a worldwide ranking nor proof of individual eligibility.

Proposal, delivery, improvement and sustained outcome differ. Group probability is not personal destiny. Fictional10/100 without A and8/100 with A over the same period yield2cases/2percentage points absolute and20% relative difference. These are not ECT/TMS data. Ask which outcome, period, population and comparison are meant; understanding does not require independently calculating a table.

ECT discussions include possible memory difficulties; some memory gaps can persist. Promise no universal reversibility or loss of personal identity. Observation differs from established cause; exhausting home tests are not required for a concern to matter. Current danger needs local emergency help, not philosophical certainty or completed notes.

Questions and conditions

What exact method/form, why now, and what evidence fits this person? What is known about benefits, harms and alternatives, including missing appropriate help? Who assesses physical health, prepares, delivers and supports recovery? How are problems reported between visits, what gets reviewed and who remains responsible afterwards?

Consent concerns an understood proposal; a signature does not replace understanding. Another accessible explanation and another professional opinion are different resources. Consent/refusal/representation/emergency exceptions/advance wishes require local clarification; diagnosis and speech speed do not automatically determine them. Communication help does not transfer choice to a relative. No universally safe delay or export of United States approval is assumed.

Fictional Marina illustrates fear and memory questions; Oleg, an understood method with inaccessible attendance. Consider consultations/preparation/repeated journeys/work/cost/accompaniment/recovery/caregiving. Do not record a missing helper as available. Spoken questions are acceptable; devices and safe relatives are not assumed. Do not change medicines, food before anaesthesia or restrictions from another person's story; the treatment team gives specific instructions.

Essential care needed now for a child/dependent adult requires an actual capable adult/professional. A child is no substitute responsible escort or observer of a parent's recovery. Relatives' help is feasible and agreed; violence requires independent protection. A conversation is not yet accessible care. Naming an open question and functioning contact is a modest result, not proven recovery.
